Rashes affect up to one-third of patients with M. pneumoniae respiratory tract infection. The most frequent rash is non-specific exanthem in which short-lasting red patches appear on trunk and limbs. These areas of erythema clear by themselves and do not require specific treatment.

Mycoplasma-induced rash and mucositis is one of the more recently discovered extrapulmonary manifestations of Mycoplasma pneumoniae infections. The study is the first systematic review of the MIRM literature since the introduction of the diagnosis in 2014.

Mycoplasma pneumoniae can cause mucocutaneous eruptions, including a new entity from the epidermolytic dermopathies spectrum, called M. pneumoniae-induced rash and mucositis (MIRM). The mucocutaneous disease associated with M. pneumoniae presents predominant mucositis, with scarce or absent cutaneous involvement.

Signs and symptoms of Mycoplasma pneumoniae–Induced Rash and Mucositis (MIRM) include: - Fever, cough, and general discomfort about a week before the rash appears. - Rashes primarily in areas like the mouth (94% of cases), eyes (82%), and genitals (63%).

Because of the distinct morphology, mild disease course, and potentially important clinical implications regarding treatment, we propose a revision of the nomenclature system and suggest the term "Mycoplasma-induced rash and mucositis" for these cases.

Clinical and histopathologic findings were consistent with Mycoplasma pneumoniae-induced rash and mucositis. The patient was treated with a 7-day course of azithromycin and prednisone. Complete resolution of the mucocutaneous lesions and respiratory illness was observed on follow-up examination.

Mycoplasma pneumoniae infection is associated with extrapulmonary complications, including mucocutaneous eruptions. These eruptions, which have been termed either “Stevens-Johnson syndrome” or “erythema multiforme” in the literature, may differ from drug-induced Stevens-Johnson syndrome or viral-associated erythema multiforme.

MIRM is an uncommon entity characterized by prominent mucositis, usually with sparse cutaneous involvement. The diagnosis of MIRM can be challenging due to the lack of awareness amongst clinicians.
