In the forefoot, partial amputation of the metatarsals preserves the length of the stump and, thus, minimizes the loss of weight-bearing surface. The resection of metatarsal and midfoot bones without removing the toes, called a "hidden" amputation, is more acceptable to the patient who does not feel as if he/she has become an amputee.

Transmetatarsal amputation (sometimes called a forefoot amputation) is indicated for gangrene or infection affecting several or all toes. It is essential that the plantar skin is healthy as the incision uses a total plantar flap.

Surgical approaches for forefoot osteomyelitis include amputation with immediate wound closure or resection followed by either staged re-resection and wound closure or local care of the open wound for secondary healing.

The primary indication for TMA is the presence of a nonviable distal forefoot. The multiple causes include, but are not limited to, ulceration, failed toe and isolated ray amputations, peripheral vascular disease, trauma, embolic phenomona, frostbite, congenital deformities, and tumors.

Amputations are caused by both medical conditions and trauma. Medical conditions resulting in amputation are often those that restrict blood flow to extremities, usually by atherosclerosis forming in the arteries.
