At its simplest, integrated care is an approach to overcome care fragmentations, especially where this is leading to an adverse impact on people’s care experiences and care outcomes.

Over the past decade many definitions, concepts and theories have emerged explaining and defining what integrated care is and what the main building blocks for the successful integration of services are.

In the following sections, we will outline the pillars of our framework: (1) the key elements of primary care, (2) the dimensions of integrated care, and (3) the combination of the key elements of primary care and integrated care.
