To overwrite original portraits: Go to Applications > Baldur's Gate > Game Data > 00777. Then right-click on Baldur's Gate and go to 'Show package Contents'. Then > Content > Resources and the override folder should be there. Otherwise (to add portraits for the 'custom' button), do to Documents > Baldur's Gate Enhanced Edition.

This was a crazy effort: 200-500 generated images per NPC plus extra variants, more than 10k total portraits, nearly 1 out of 5 was quickly preselected, and then iteratively filtered by checking for typical artifacts of AI-generated imagery.

Go to My Documents -> Baldur's Gate - Enhanced Edition and create a folder named 'portraits' and put them there. Update 1.2 made using custom portraits more flexible, so they can be any size as long as they don't exceed 1024 pixels in either dimension.

There's a readme in the base BG2 folder that explains custom portraits. I just got a new computer so I don't have BG2 installed or I'd just copy and paste that portion. Basically it's any two (one for the larger portrait in the character menu and one for the smaller sidebar) specifically sized bmps placed in the portraits folder.
