Is the response "I am fine, thank you. And you?" outdated?
@HaL: Of course you can argue that; you just have to provide the situation. If I'm talking to my friends, family, or co-workers in day-to-day speech, I can say it. If I hear it on television or read it in a book, I readily understand it. If I am writing a formal document or giving a talk, I avoid it. Even dictionaries acknowledge this usage.
"And you?" or "And yourself?" as response to "How are you?"

The second sentence has a silent you in it, referring back to the fact that it was the original person who asked first and is being thanked. I am well, thank you, and you? I am well, thank you, and (you) yourself? However, asking, "How are you?"
