The Kenalog could be given without the addition of the lidocaine, it would just be extremely uncomfortable for the patient.

2012年1月26日 · J2000 was deleted in 2005 and was deemed non billable for injections in 01 I believe, the article sited above is from 02 so I think it was 01 it may have been earlier, but the J2000 was deleted from the HCPCs II book in 04 effective in 05. The only billable code for lidocaine is the IV route.
