At the foramen magnum anteriorly, there is an extra-axial mass with low signal intensity on T1 weighted images and mildly hyperintense on T2/FLAIR weighted images. The mass reveals homogeneous enhancement on post-contrast injection with dural tail. There is significant mass effect and compression of the medullo-cervical region posteriorly.
